Print Personal Data Sheet (one for each employee) (click here)
Can be completed on-line and e-mailed to Shield Medical Inc. (especially useful for adding new employees)
Mail original receipts to Edmonton Head Office along with a company cheque made payable to Shield Medical Inc. for initial set-up fee of $239.00 (includes tax)
